What is hair transplant?

For hair transplantation, the patient's scalp is used to avoid the problem of rejection. And this means that the density of hair that will be transplanted is determined by the capabilities of the patient.

Methods of hair transplantation

In modern medicine, several methods of hair transplantation are used and exist.

Strip method or operating transplant. This is a surgical operation, in which all available hairs and follicles are distributed throughout the affected area.

Non-surgical method. This is a seamless method, which is carried out with the help of special equipment and without surgical intervention , allows you to use hair from all areas of the patient's body.

Combined method. Combined hair transplantation allows you to combine the technology of the first two methods - the most effective, but an expensive method.

Consequences of hair transplantation

The fact is that hair transplantation with the help of any method does not go unnoticed.

The solution of the problem - hair transplantation - was found in the first half of the 20th century, but since then people have not ceased to doubt the need for a procedure. Is it safe? Is the effect really going to be what the clinic promises? How long will hair take root, and, finally, how much will the transplant cost? According to statistics, the most frequent cause of hair loss is androgenic (or androgenetic) alopecia. Scientists can not yet fully explain the mechanism of its development, but it is already clear that the main effect on hair loss is the enzyme 5-alpha-reductase - it is "to blame" that testosterone changes the passive form to active and becomes dihydrotestosterone (DHT). DHT works on some receptors in hair follicles, thereby inhibiting their activity - making the cycle of hair growth shorter, and the hair itself - thinner. The process of "falling asleep" and then falling out of the hair follicles can take from several months to several years - after all, the hair on the vertex (where the enzyme 5-alpha-reductase is most active) simply ceases to grow.

There is also a question that absolutely everyone asks: How painful is the procedure?

The procedure is almost painless. Specialists perform hair transplantation manually. The procedure is performed using local anaesthesia and lasts from 2 to 8 hours - during which the patient does not feel unpleasant sensations, usually, there is not even an easy tingling. According to the statistics of our clinic, more than 98% of the transplanted hair takes root. Hair looks natural - including at the donor site, at the back of the head: during the transplantation of the hair, and accordingly, there are no scars after the procedure.

Of course, there are absolute restrictions, but there are not so many: for example, surgery is not recommended for patients who have bleeding disorders, and also for those who do not tolerate anesthesia or its individual components.

Relative contraindications for hair transplantation are considered to be high blood pressure, cardiovascular diseases and heart surgery, cancer (in this case, a permit for hair transplant from the treating doctor is required) and diabetes mellitus (there is a risk of postoperative infections and difficulties with the engraftment of the hair).

The HFE technique allows hair to be transplanted to people suffering from diabetes mellitus. But such patients are recommended to be under the supervision of a therapist, and, in addition, there are some restrictions on the number of transplanted follicles.

Hair transplantation is not recommended for patients with skin diseases - if these diseases are in acute stage, as well as for patients suffering from some mental disorders: trichollomania (man is prone to uncontrollably pulling his hair out), obsessive-compulsive disorder (obsessive thoughts and related behaviour) and dysmorphophobia (hostile attitude towards one's body and everything connected with it).

Although there are no absolute contraindications to hair transplantation, there are a number of relative contraindications, except for the same as in all cases of non-vital operations: bleeding disorders and intolerance to anesthesia are common absolute contraindications to hair transplantation.

Age: follicle transplantation is not recommended for people younger than 15 years. Hair loss in adolescence is likely to lead to extensive baldness, and at this age, it is impossible to accurately determine the area of the lesion, which may in the future lead to the fact that in the donor area the hairline may become insufficient for subsequent necessary transplants.

Mental diseases: hair transplantation is not performed with a number of mental disorders:

  • Trichotillomania - a disease in which a patient suffers from an uncontrolled desire to wrest hair from himself;
  • The obsessive-compulsive disorder characterized by repetitive, obsessive thoughts and behavior associated with them;
  • Dysmorphophobia is a mental disorder that involves a distorted view of one's body.

If in the first case it is obvious that hair transplantation in the stage of exacerbation of this disease is impossible, the two subsequent diseases are contraindications to transplantation due to the predicted state of patient dissatisfaction with the results of the procedure at any outcome of the operation.

Diabetes mellitus: patients with diabetes mellitus are at greater risk of postoperative infections, so there are a number of clinics that claim that hair transplantation is contraindicated for this group of people. The HFE technique allows hair transplantation in diabetes mellitus, but with a restriction on the number of transplanted follicular associations.

Hair transplantation is not possible if the patient suffers from skin diseases at the stage of exacerbation. There is a possibility of holding in the period of remission or with complete cure.

People with high blood pressure, diseases of the cardiovascular system, especially with cardiac implants, with oncological diseases, should get a consultation with the doctor before deciding on hair transplantation.

With the use of an operating or partially operating procedure for hair transplantation, the following complications are possible, which are virtually excluded when using the HFE technique:

  • Postoperative numbness associated with damage to nerve endings in the donor area or in the area of transplantation, which in the vast majority of cases is temporary.
  • Scarring, inevitably arises during hair transplantation by the surgical method, according to which not separate follicles are taken from the healthy occiput, but whole skin areas (band, bundles). The number of scars and appearance depends on the skill of the staff and the individual properties of the patient's skin.
  • Rejection: there is always a risk that the operation will end with the rejection of the implant. Predict the reaction of the body is impossible, but such a reaction does not bring physical harm to itself.

Hair transplantation is a very accurate procedure, requiring a qualified approach; as with any surgical intervention, it is associated with risk in the event that it is performed by under-qualified personnel in inadequate conditions. You can avoid this by choosing certified medical clinics.

The practice of hair transplantation has more than half a century. For the first time, questions on transplantation were raised in Japan as far back as 1939, and already in the 60s. The first operations were carried out in the last century. Such operations became a revolution in medicine and cosmetology, allowing millions of people on the planet to struggle with the "flaws" of their appearance. Hair transplantation is solved not only by the problems of people suffering from baldness for some natural reasons but also helps to hide scars, traces of burns, suffered injuries, unsuccessful operations, including face lift (which is now very popular), or just someone, then wants to restore the density of hair on the head and the facial part of it (moustache, eyebrows, beard), or certain diseases. To date, there are two main methods for hair transplantation - operating and non-surgical.

What is hair transplant and how is it performed?

What is a hair transplant or transplant? Many today heard about this procedure, but very few people understand what is at stake. Actually, both operational and non-surgical methods of transplantation include two main steps: extraction of hair from the donor zone and setting the hair in the hair loss zone. The difference between methods is in the methods of extraction and staging.

Operational methods of hair transplantation.

In the first case, cutting tools (scalpels) are used. With the help of a scalpel, a strip of skin is cut from the donor area (the back of the patient), from where the hair used for the graft is taken. These transplants, in turn, are transplanted with tweezers into the area of hair loss in the incisions, which again are formed with the help of a scalpel. At the same time, a scar 15-20 (25) cm long remains on the donor zone for a lifetime. It is impossible to achieve a natural hair density in this way, even when carrying out several operations.

Non-surgical methods of hair transplantation

The non-surgical method of seizing hair for the first time begins to be applied in the 90's. last century. This technique of partially non-surgical or otherwise seamless hair transplantation, known as FUE machine (in Russia - TFI), implies that only the first stage - removal of transplanted hair - grafts are performed without surgery. At the same time, the withdrawal itself is made using rotating piercers, whose diameter (1.8-5 mm) significantly exceeds the diameter of the hair and as a consequence is unavoidable for life, as well as after surgery, there are traces - a lot of shallow scars on the donor-zone. Grafts are also transplanted with tweezers into pre-cuts made by the scalpel or less often by punctures. The density of hair does not exceed the density achieved with the operating method. In both cases, it is about 50 hair per 1 cm2. Moreover, in the second case, the re-operation is impossible because of serious traumatization of the donor-zone, which does not allow achieving a greater density of hair. Of course, such achievements are far from ideal, which made the researchers conduct more in-depth research in search of a solution to the problem.

New non-surgical method of hair transplantation

The solution was found in the early 2000s when scientists noticed that the hair grows in groups that have up to 3-5 roots, that is, the follicular union of the hair. Based on this discovery, a revolutionary method of hair transplantation was developed, combining non-surgical manual removal (FUE hand) and non-surgical hair transplantation (FUI). This newest technique is known in Russia as Hand follicle extraction (HFE). That is, it is implied that the whole procedure for hair removal is done by the doctor's hands, without involving any mechanisms: the donor zone is cut off at the back of the head and with the help of a special instrument, which is a microtubule (0.5-0.9 mm in diameter) the most healthy follicular associations are removed, which are already ready transplants. Such withdrawal allows to avoid additional traumatization of the hair roots, because they do not need to be cut into grafts and (!) most importantly, skin damage is minimal, as the extraction of transplants is performed in the upper layers of the skin and, in fact, does not affect the nerve endings. Direct hair transplantation is performed using a unique micro-instrument - Choi implanter, which allows to avoid preliminary cuts in the hair setting zone: the hair follicles descend to a given depth, at a natural angle of inclination and hair growth, which also allows to achieve a natural density of hair transplanted (up to 80 on 1 cm2 in the first stage, and up to 120-140 hair per 1 cm2 on the second). The diameter of the implant needle is equal to the diameter of the micro-patch. The entire procedure takes about 8 hours using local anaesthesia. An absence of surgical intervention excludes any postoperative complications - no swelling, no bruises, no scars, no prolonged loss of skin sensitivity. Inevitable microscopic wounds (reminiscent of punctures from a medical injection) heal without a trace in 3-5 days. During the year the transplanted hair grows completely and will never fall out (in the absence of known adverse conditions: nervous breakdowns, radiation exposure, a chronic shortage of macro- and microelements, vitamins, etc., which in real life happens very rarely).
